A hairline hair transplant at Hair Transplant Belfast addresses frontal recession using the FUE technique to restore a natural hairline position and density. The hairline is one of the most visible aspects of hair restoration and its design requires precision, conservatism, and a clear understanding of how the face will age.

Hairline Hair Transplant in Belfast

Hairline recession is typically the first visible sign of male pattern hair loss. As the frontal hairline retreats and the temples thin, the face changes shape significantly. A hairline transplant can restore the hairline to a position that suits the patient’s face, frames the features correctly, and looks natural at the current age and in the decades ahead.

Follicular unit extraction removes individual follicles from the back of the scalp and transplants them to the receded area. For patients with existing hair in the frontal zone who need coverage around it, the DHI technique is sometimes preferred. At the hairline edge specifically, only single-hair follicular units are used. Two- and three-hair grafts placed at the very front create an unnatural density line. The natural hairline has a gradual transition from single hairs at the edge to fuller density behind it, and recreating this requires careful graft selection and placement.

Hairline Design

Before the procedure, Dr. Aziz Elgindi (GMC: 7450896) or Dr. Mohammed Ditta (GMC: 7084372) will design the new hairline with the patient. Several factors are considered: the current position of the recession, the patient’s age and likely future hair loss trajectory, the facial structure, and the patient’s expectations. A hairline placed too low for a man in his 30s with ongoing hair loss will look out of place in his 50s if further loss occurs behind it.

We aim for a hairline that looks natural today and ages well. Patients who want their teenage hairline restored are usually advised that a more conservative placement will serve them better long-term. Patients who prefer to avoid shaving the donor area may want to discuss FUT, though this is less common for hairline-only procedures.

How Many Grafts Are Needed?

A hairline restoration typically requires 1,200 to 2,500 grafts depending on the degree of recession and the desired density. Significant frontal recession combined with temple thinning may require more. The graft count is confirmed at consultation after a clinical scalp assessment.

If the crown is also thinning, combined treatment planning is important. See our page on crown hair transplant for information on addressing that area.

Understanding Hairline Recession and the Norwood Scale

Male pattern hairline recession follows a broadly predictable sequence described by the Norwood-Hamilton scale. Norwood Type I and II represent early recession around the temples with minimal frontal retreat. Types III and IV show more pronounced temporal recession and thinning behind the hairline. Types V through VII describe progressive loss extending from the front to the crown, eventually leaving only a horseshoe of hair around the sides and back.

Understanding where you sit on this scale matters not just for planning the current procedure, but for thinking about what further loss is likely. A patient at Norwood III in his late 20s may reach Norwood V or VI within a decade if hair loss follows a typical pattern for his family history. Planning a hairline transplant without accounting for this trajectory can result in an isolated front hairline sitting above advancing baldness within a few years.

This is not a reason to avoid surgery. It is a reason to plan it carefully, at the right position and with enough conservatism to remain appropriate as the surrounding hair continues to change. Medical management with finasteride and minoxidil alongside a transplant can slow or halt further recession in untreated areas.

What Makes a Natural Hairline?

A natural hairline is not a straight line. It has micro-irregularity, meaning individual hairs at the very edge are slightly uneven in their placement rather than sitting in a uniform row. It has a gradual transition from single, fine hairs at the front edge to fuller density a few centimetres behind. It frames the face differently at different angles, which a perfectly symmetrical or geometrically precise hairline does not.

At Hair Transplant Belfast, only single-hair follicular units are used at the hairline edge. Two- and three-hair grafts are placed further back in the frontal zone to build density, but the leading row is always single hairs placed at a very low angle to create the soft, natural transition that distinguishes a good transplant from an obvious one.

The temporal recession also matters. For many patients with frontal hairline recession, the temples have receded too. Restoring just the frontal line without addressing the temples can leave a shape that does not match the natural hairline of the patient’s age or facial structure. Temple work typically adds 200 to 400 grafts to the session.

Hairline Transplant Without Shaving: The FUT Option

Some patients cannot take time off work with a shaved donor area and want to keep the procedure as discreet as possible. For these patients, FUT is worth discussing. With FUT, only the narrow donor strip at the back of the scalp is shaved, and the surrounding hair immediately above and below the strip conceals it. The recipient area in the frontal zone does not need to be shaved.

This option involves a linear scar at the donor site, which is the trade-off. For patients who consistently wear their hair at a length of grade 3 or above at the back, this scar is not visible and the recovery is considerably more discreet than a fully shaved FUE procedure.

FAQs

The appropriate hairline position depends on your facial proportions, age, and the likely progression of your hair loss. A hairline placed too low for your face or too aggressive for your age can look unnatural in later years if surrounding hair continues to thin. Your surgeon will propose a hairline position at consultation and explain the reasoning. You have final say on the design.

Yes, when executed correctly. The hairline edge uses only single-hair follicular units placed at a very low angle to create a soft, gradual transition. Two- and three-hair grafts are positioned further back to add density. This graduated approach is what produces a natural-looking result.

Transplanted hairs at the hairline are resistant to the DHT that causes male pattern hair loss, because they come from the permanent donor zone. However, hair behind and around the transplanted area may continue to thin if the underlying hair loss is progressive. Your surgeon will discuss this risk at consultation and may recommend medical treatments to slow ongoing loss in untreated areas.

Yes. Hairline refinement procedures are possible where additional grafts are placed to improve density or adjust the edge. Any refinement must account for the remaining donor supply. Over time, as hair loss progresses, patients sometimes choose to address the crown or midscalp in subsequent sessions.

Transplanted hairs shed at two to four weeks. New growth begins from around three months. The hairline continues to fill in and mature up to 12 to 18 months. The final result is assessed at that point.

A hairline transplant specifically addresses the frontal hairline and temples. A full hair transplant may also cover the midscalp and crown. For patients with recession limited to the frontal zone, a hairline transplant is often the right scope. For more extensive hair loss, a combined approach covering more of the scalp may be recommended.

It was an outstanding experience from start to finish. Naturally, I was unsure on which organisation to choose for the procedure, but HSHTC came across as professional and caring, as well as having a wide range of availability. There are negative reviews on all businesses, and this drives customers like myself away. However, something told me this would likely be someone with unrealistic expectations, or not reading into the procedure as a whole. I chose to ignore the negatives and read the positives. Sara the coordinator was absolutely amazing! She outlined everything and made it clear of what is expected and what is not. After a great consultation, I was passed to the next stage of meeting surgeon Dr Ditta. He presented extremely knowledgeable and passionate about what he does, so I decided to go ahead with the procedure. The surgery day was expected to be a long, but rewarding day. It started with having to shave my head (mandatory requirement for this procedure). I’d say to anyone the thought is worser than the event, and to just do it for the positives that will follow. Dr Ditta and his team were extremely caring and sensitive, especially with briefing before any painful situations pre anaesthetic. After that, I was allowed to top up when requested and leave them to do what they do best. The day as a whole was enjoyable and interesting to speak to the team to learn more about the procedure and get to know them. Lunch was provided (yay). It was lovely of them to offer, and you definitely need to eat mid procedure to keep energy levels. At the end of the day, I was given clear instruction of how to have the best success rate. It was through a video and in person. If you have questions, ask as it’s what the team is there for. The first few days of recovery can be quite awkward and uncomfortable, but I’d recommend booking 2 weeks off work to fully recover and let the grafts set. Once you get use to the sleeping situation, it’s easy and a lovely break from reality (put pillows below your knees and stack below head for pure comfort!). The aftercare products was extremely useful and easy to use. The grafts will fall out after a few weeks, it’s expected and you’re told this plenty of times!! Having a healthy lifestyle makes your hair grow quicker and thicker, so swap your diet to good healthy hair foods. When your transplanted hair grows back, it’s an amazing feeling, but only the beginning. As the months pass by, you’ll notice a massive improvement. During this process, you’ll have regular aftercare and the chance to raise any concerns. I personally didn’t have any issues and was just grateful to still be in contact and told everything is going as it should. I’m 6 months post surgery and I’m seeing the benefits hugely, and it’s not even reached maximum results yet, as this is one year. Thanks for an amazing service!

Having given some time and thought about having a Hair Transplant, I contacted Harley street hair transplant clinics. The consultant (Sara) explained everything regarding the procedure with Dr.Ditta and the technicians. From the beginning, Dr.Ditta explained the days events and how the procedures would be done beginning with taking a photo of my head and showing me where the grafts would be taken from and where they would be placed. The procedures would be done and completed in two parts, with a lunch break in between. Once completed, Dr.Ditta went through the aftercare, explaining what to do each day for 10 days. I was given capsules, shampoo, sponge, neck pillow for sleeping with and information regarding how to look after my hair. Dr. Ditta and his team were welcoming and friendly ensuring I was looked after throughout both parts of the procedure. Sara maintained regular contact to make sure recovery was going well. The results were great, patience is needed as the grafts start to grow, and then you can see the results. Extremely happy with everything, from start to finish.

Would highly recommend! Everything went very smoothly, communication was excellent throughout. I travelled from Edinburgh to Manchester for the procedure and all travel and accommodation costs were covered. Dr Ditta explained everything clearly and delivered clear and detailed post-op instructions. I was delighted with the whole experience and results so far are very promising!

Overall, my experience at the clinic was positive. Dr. Ditta was exceptional — he made me feel comfortable and at ease throughout the procedure, and his professionalism was truly reassuring. However, I was disappointed with one member of the team who, in my opinion, did not fully follow health and safety protocols. She had her hair down without a surgical cap and was using her phone while wearing gloves, then proceeded to touch my hair without changing them. This raised concerns about hygiene and cross-contamination. Her manner was also not very pleasant, which made the experience less comfortable. Although I was initially worried about the risk of infection, I felt more at ease by the end of my visit. I hope the management reviews this matter to ensure that strict hygiene and professional standards are consistently maintained. Aside from this, I was very satisfied with Dr. Ditta’s care and look forward to seeing my results.

Dear Team. Following my hair transplant last Saturday I would like to take this opportunity to thank all of the team, for making my journey as pleasant as possible. From the first consultation, to having my hotel booked, to the procedure itself, I couldn’t have been more impressed. Finally a big thank you to Doctor Ditta and the team, who were so kind when I felt unwell, making me drinking chocolate and reassuring me throughout the process. Add to this, their expertise and professionalism, what more can one ask for? I look forward to the final part of my hair transplant next year. Once again a big thanks to you all. Kind regards Keith Yates

Before the procedure I had online and face to face consultations with no obligations. These gave me all the information I needed and really put me at ease and gave me the confidence to proceed. Everyone was really knowledgeable and friendly and clearly very experienced. Booking the procedure was quick and easy and I got a date that suited me. The procedure itself went really smoothly I was able to put headphones in and listen to podcasts and was very comfortable. Afterwards I was given really detailed notes on what to do and what would happen and everything went exactly as planned. It was so reassuring knowing that advice was only a text message away and responses were always immediate. I'm two weeks post procedure now and everything has gone back to normal and the recovery is going just as planned. I'm so glad I did this and chose Harley Street Hair Transplant!

Great clinic with friendly and experienced team. I wanted my surgery done in the UK and found the clinic on Google. Process started with virtual consultations with patient advisor and then with the surgeon. FULL costs were given up front, with good finance options available. On the day I designed a new hairline with the surgeon and ended up getting about 1600 grafts iirc, which was good value for money. Clear pre- and post-op instructions given and recovery has gone as expected. Now waiting patiently for regrowth :) - Grant
