Hair transplant surgery for transgender patients addresses two distinct presentations: hairline feminisation for trans women, and beard transplantation and hairline masculinisation for trans men. At Hair Transplant Belfast, we approach transgender hair restoration with the same clinical care as all our procedures, with specific attention to the anatomical and hormonal differences relevant to each patient’s situation.

 

Transgender Hair Transplant in Belfast

Transgender hair restoration is not a single procedure but a category that covers several different goals depending on the patient’s gender journey and the specific changes they want to address.

For trans women: The male hairline sits higher and has more pronounced temporal recession than a female hairline. Hairline feminisation lowers the frontal hairline and fills in the temples to create a curved, softer hairline shape consistent with female hair patterns. The procedure uses FUE to transplant follicles from the donor area to the frontal zone and temples, with FUT considered for patients with more advanced recession requiring higher graft counts. Crown thinning in trans women may also need to be addressed if male-pattern baldness progressed before transitioning. The graft count depends on the degree of existing recession, which may be significant if the patient experienced male pattern baldness before transitioning.

Patients on oestrogen therapy who have stopped any active hairline recession are the best candidates. Hormone-induced stabilisation is important before surgery, as a receding hairline at the time of the procedure may continue to change in the months following.

For trans men: Testosterone therapy typically stimulates beard growth and can produce male-pattern hairline characteristics over time. Patients who have been on testosterone for several years and have not developed the beard density they want, or who have a hairline that has not responded to hormone therapy, may benefit from surgical intervention. Beard transplantation using the DHI technique is the primary option for trans men wanting facial hair, and eyebrow restoration is also available for trans women wanting denser brows.

 

Consultation

Transgender hair restoration requires an extended consultation. We take a full history including hormone therapy timeline, surgical history, and current goals. For trans women, we assess the degree of male-pattern recession, the existing hairline position, and the donor supply. For trans men, we assess facial hair distribution and what beard density is achievable given the available donor area.

Trans men who have developed hair loss on testosterone are assessed on the same clinical basis as male pattern baldness. All patients should be stable on hormone therapy for at least 12 months before surgery is considered, ideally longer. This ensures the hair loss pattern is stable and the surgical plan reflects the patient’s long-term hair situation rather than a transitional state.

 

What Makes a Feminine Hairline: The Anatomy

Male and female hairlines differ in several specific ways. A male hairline typically sits higher on the forehead, has pronounced temporal recession forming an M-shape, and has a flatter, more horizontal frontal border. A female hairline sits lower, has a rounded arch that frames the forehead, retains hair in the temples with a softer curve rather than sharp recession, and often has a slight widow’s peak at the midline.

Hairline feminisation does not simply mean lowering the front of the hairline. It means reshaping the entire perimeter, including filling the temporal recession, softening the corners and creating the rounded arch profile that characterises a feminine hairline shape. The amount of reconstruction required depends on how much male-pattern recession has occurred. A trans woman who transitioned before significant male-pattern loss had progressed may need only minor refinement. A patient who experienced Norwood IV or V recession before transitioning requires a more substantial reconstruction covering the temples, frontal zone and possibly the midscalp.

Only single-hair follicular units are used at the very edge of the new hairline. The transition from the hairline forward to bare forehead must be gradual and natural, with micro-irregularity at the edge rather than a uniform line.

 

Hair Restoration for Trans Men in Belfast

For trans men, testosterone therapy typically stimulates beard growth and can cause androgenetic alopecia in those with genetic predisposition. The hair loss pattern on testosterone follows male pattern progression, usually beginning with temporal recession and advancing toward the crown. This progression can happen relatively quickly in some patients.

Trans men who want to preserve scalp hair while on testosterone have the same medical options as cisgender men. Finasteride reduces DHT activity and can slow loss, though it may affect testosterone-related changes in other ways and requires individual medical review before use. Minoxidil is an alternative with no hormonal effects.

For trans men who have experienced hair loss on testosterone and want restoration, the procedure is assessed on the same clinical basis as male pattern baldness. Donor density, the extent of loss, and future trajectory are all considered. FUE is typically the technique of choice for scalp work.

Beard transplantation for trans men uses the DHI technique to place follicles at the precise angles required for natural facial hair. The best time for this procedure is after testosterone has had time to stimulate what natural beard growth it will produce. Most surgeons recommend waiting two to three years on testosterone, as surgery before the natural growth has reached its plateau can result in an overly dense result if the testosterone-driven beard continues to develop after the transplant.

 

Preparing for Surgery: Timing and Candidacy

The most important preparation step is stabilising on hormone therapy before surgery. For trans women, oestrogen therapy slows and often stops male-pattern hairline recession. Surgery should be delayed until the hairline has been stable for at least 12 months, and ideally 18 to 24 months, following the start of oestrogen. A hairline that is still changing at the time of surgery will not hold the same design once the procedure is complete.

For trans men, similar logic applies to beard work. Testosterone-driven beard growth can continue for two to three years after starting HRT. Surgical beard augmentation is best timed after the natural testosterone response has reached its limit, so the surgeon is designing around a stable baseline rather than an area still changing.

All patients are also assessed for the general health factors that affect any surgical candidate: scalp health, chronic conditions, medications, and realistic expectations about the achievable result. Trans patients who have undergone other gender-affirming procedures should mention this at consultation, as some surgical histories are relevant to planning.

In patients who experienced significant baldness before transitioning, hair transplantation addresses recession that hormone therapy alone will not reverse. A minimum of 12 months on hormone therapy is recommended before seeking a surgical consultation.

It depends on the degree of existing recession and the desired result. Mild recession requiring softening of the temples and minor lowering of the frontal hairline may need 1,200 to 1,800 grafts. More advanced recession requiring significant reconstruction of the frontal zone and temples can require 3,000 or more. The graft count is confirmed after a clinical assessment at consultation.

Testosterone therapy can cause or accelerate male-pattern hair loss in trans men. For trans men who want to preserve or restore scalp hair while on testosterone, this must be discussed at consultation. Medical management alongside any surgical procedure is important. For trans women who have stopped testosterone and are on oestrogen, the transplanted hairline is typically stable.

It was an outstanding experience from start to finish. Naturally, I was unsure on which organisation to choose for the procedure, but HSHTC came across as professional and caring, as well as having a wide range of availability. There are negative reviews on all businesses, and this drives customers like myself away. However, something told me this would likely be someone with unrealistic expectations, or not reading into the procedure as a whole. I chose to ignore the negatives and read the positives. Sara the coordinator was absolutely amazing! She outlined everything and made it clear of what is expected and what is not. After a great consultation, I was passed to the next stage of meeting surgeon Dr Ditta. He presented extremely knowledgeable and passionate about what he does, so I decided to go ahead with the procedure. The surgery day was expected to be a long, but rewarding day. It started with having to shave my head (mandatory requirement for this procedure). I’d say to anyone the thought is worser than the event, and to just do it for the positives that will follow. Dr Ditta and his team were extremely caring and sensitive, especially with briefing before any painful situations pre anaesthetic. After that, I was allowed to top up when requested and leave them to do what they do best. The day as a whole was enjoyable and interesting to speak to the team to learn more about the procedure and get to know them. Lunch was provided (yay). It was lovely of them to offer, and you definitely need to eat mid procedure to keep energy levels. At the end of the day, I was given clear instruction of how to have the best success rate. It was through a video and in person. If you have questions, ask as it’s what the team is there for. The first few days of recovery can be quite awkward and uncomfortable, but I’d recommend booking 2 weeks off work to fully recover and let the grafts set. Once you get use to the sleeping situation, it’s easy and a lovely break from reality (put pillows below your knees and stack below head for pure comfort!). The aftercare products was extremely useful and easy to use. The grafts will fall out after a few weeks, it’s expected and you’re told this plenty of times!! Having a healthy lifestyle makes your hair grow quicker and thicker, so swap your diet to good healthy hair foods. When your transplanted hair grows back, it’s an amazing feeling, but only the beginning. As the months pass by, you’ll notice a massive improvement. During this process, you’ll have regular aftercare and the chance to raise any concerns. I personally didn’t have any issues and was just grateful to still be in contact and told everything is going as it should. I’m 6 months post surgery and I’m seeing the benefits hugely, and it’s not even reached maximum results yet, as this is one year. Thanks for an amazing service!

Having given some time and thought about having a Hair Transplant, I contacted Harley street hair transplant clinics. The consultant (Sara) explained everything regarding the procedure with Dr.Ditta and the technicians. From the beginning, Dr.Ditta explained the days events and how the procedures would be done beginning with taking a photo of my head and showing me where the grafts would be taken from and where they would be placed. The procedures would be done and completed in two parts, with a lunch break in between. Once completed, Dr.Ditta went through the aftercare, explaining what to do each day for 10 days. I was given capsules, shampoo, sponge, neck pillow for sleeping with and information regarding how to look after my hair. Dr. Ditta and his team were welcoming and friendly ensuring I was looked after throughout both parts of the procedure. Sara maintained regular contact to make sure recovery was going well. The results were great, patience is needed as the grafts start to grow, and then you can see the results. Extremely happy with everything, from start to finish.

Would highly recommend! Everything went very smoothly, communication was excellent throughout. I travelled from Edinburgh to Manchester for the procedure and all travel and accommodation costs were covered. Dr Ditta explained everything clearly and delivered clear and detailed post-op instructions. I was delighted with the whole experience and results so far are very promising!

Overall, my experience at the clinic was positive. Dr. Ditta was exceptional — he made me feel comfortable and at ease throughout the procedure, and his professionalism was truly reassuring. However, I was disappointed with one member of the team who, in my opinion, did not fully follow health and safety protocols. She had her hair down without a surgical cap and was using her phone while wearing gloves, then proceeded to touch my hair without changing them. This raised concerns about hygiene and cross-contamination. Her manner was also not very pleasant, which made the experience less comfortable. Although I was initially worried about the risk of infection, I felt more at ease by the end of my visit. I hope the management reviews this matter to ensure that strict hygiene and professional standards are consistently maintained. Aside from this, I was very satisfied with Dr. Ditta’s care and look forward to seeing my results.

Before the procedure I had online and face to face consultations with no obligations. These gave me all the information I needed and really put me at ease and gave me the confidence to proceed. Everyone was really knowledgeable and friendly and clearly very experienced. Booking the procedure was quick and easy and I got a date that suited me. The procedure itself went really smoothly I was able to put headphones in and listen to podcasts and was very comfortable. Afterwards I was given really detailed notes on what to do and what would happen and everything went exactly as planned. It was so reassuring knowing that advice was only a text message away and responses were always immediate. Great clinic with friendly and experienced team. I wanted my surgery done in the UK and found the clinic on Google. Process started with virtual consultations with patient advisor and then with the surgeon. FULL costs were given up front, with good finance options available. On the day I designed a new hairline with the surgeon and ended up getting about 1600 grafts iirc, which was good value for money. Clear pre- and post-op instructions given and recovery has gone as expected. Now waiting patiently for regrowth :) - Grant
